Cell Phone/iPad Accountability and Policy Acknowledgement

Purpose:

This article outlines the company’s cell phone and iPad accountability policies, ensuring employees understand their responsibilities and the company’s expectations for handling issued equipment.


Policy Overview:

Responsibility:

  • As the user of company-provided cell phone and iPad equipment, employees are responsible for:

    • Using the equipment carefully and ensuring its security at all times.

    • Guarding against loss or theft.

  • All cell phone and iPad equipment remains the property of the company and must be returned upon request or at the end of employment.

Accountability:

  1. Alterations:

    • Employees must not alter or change the equipment (software or hardware) without explicit, written authorization from management.

  2. Reporting Issues:

    • Any damage, loss, or theft of the equipment must be reported to the employee’s manager as soon as possible, but no later than the next business day.

    • If equipment is stolen, the employee is required to file a police report before replacement equipment can be issued.

  3. Negligence and Replacement Costs:

    • If negligence is determined to have caused damage or loss, the employee may be held responsible for the cost of replacing the equipment.

    • Replacement cost will be based on the company’s current hardware standards, not exceeding the original cost of the equipment.

  4. Equipment Return:

    • At the termination of employment, all equipment must be returned in good working order. Failure to do so will result in the equipment being classified as lost or damaged, and the employee may be required to pay the replacement cost.

By signing the policy acknowledgment form, employees confirm the following:

  1. Receipt of the equipment in good working condition.

  2. Awareness and understanding of the company’s cell phone and iPad policies.

  3. Agreement to abide by the outlined conditions.

    • Manager’s Signature


Cell Phone Return Process:

Upon returning company-issued equipment, the following steps must be followed:

  1. The manager must document the return date and inspect the equipment for damage.

  2. A signed copy of the return receipt will be provided to the employee as confirmation of the equipment’s return.

  • Manager’s Signature

  • Date Equipment Returned
